The following information will remain confidential and in the custody of the pastoral staff of Journey Church. The information is required for ALL persons who have ANY contact with minor children that attend ANY function of Journey Church. During the application process, and at any time during my association with Journey Church, I hereby authorize “Volunteer Select” on behalf of Journey Church to procure a full background check/or consumer report which I understand may include information regarding my character and general reputation. These reports may be compiled with information from court record repositories, department of motor vehicles, past or present employers and educational institutions, governmental occupational licensing or registration entities, business or personal references, and other source required to verify information. I understand that I may request a complete and accurate disclosure of the nature and scope of the background verification.
